Suspect leaves Allen Hospital after treatment; court did not order supervision

The Black Hawk County Sheriff's Office is asking for the public's assistance in finding 18-year old Dominick Lewis.

Sergeant Bill Locke said Lewis was arrested Friday night, but the court ordered him to receive medical treatment at Allen Hospital.

Locke said the court order did not request supervision by the Black Hawk County Sheriff's Office, and Lewis was supposed to return back to jail after the medical treatment was completed.

But according to Locke, Lewis left Allen Hospital Saturday, and has not returned to the jail.

He said they have multiple addresses listed for Lewis.

Online records indicate a warrant was issued for Lewis' arrest on January 11, 2013, after he failed to appear in court.
