Company faces fine in wake of worker's death


IOWA CITY (KWWL) -- Iowa's Occupational Safety and Health Bureau has proposed a fine for the employer of a man who fell to his death while working at a University of Iowa building in September of 2010.

Safety regulators say Swanson Glass should be fined $8,750 for not training their employees on the safe use of ladders.

The inspection of the Clive-based company was done in the wake of the 49-year old Tom Fosdick's death.  Fosdick died after falling 40-feet from a ladder while working on windows at the Boyd Law Building.

The citation says an employee used a ladder with fully-extended levelers despite a manufacturer's warning not to do so.

Swanson referred questions to parent company KAS Investment, which had no immediate comment.

Fosdick graduated from Cedar Rapids Kennedy High School and was a swimmer at the University of Iowa in 1979.
